Problem: Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Fix: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Lexus' numbers for this recall are JLI, JLJ, and JLK.

Sep 30, 2026Unknown or other I purchased from a dealership approximately two months ago. Shortly after purchasing it, the vehicle began shutting off unexpectedly. I returned it to the dealership, and they told me they could not determine what was causing the problem. About two weeks ago, I had to take it back for a second time because the issue continued. Most recently, the vehicle shut off while I was turning in the middle of the road with my children inside. This was an extremely frightening and dangerous situation, and I am afraid to drive the vehicle because I do not know when it might shut off again. The dealership has now had the vehicle for approximately two weeks and is claiming they cannot duplicate the issue.…

Aug 9, 2026Carry handle, shell, base Swivel motion has started to jam when half locked, base will show half red/half green and refuse to lock. Unlock handle becomes hard to lift to swivel the car seat back to attempt to lock it again. Iâve contacted Evenflo, but waiting to hear back.

May 22, 2026 · crash · 1 injuryAir bags We were traveling down the mountain of Mt. Nebo in Dardanelle, AR, around 1:08pm on Friday, May 22, 2026. It had been misty rain for a few hours. As we were traveling down the switchbacks of the mountain, we pressed on the brake to slow the vehicle; however, the vehicle did not slow down any. Since we were coming to the next curve in the switchback, the decision was made to drive the car into a tree vs. risking flipping the car over the side of the mountain. When the vehicle collided with the tree, none of the air bags deployed. The point of impact was the front driver corner of the car. The car was traveling around 20 MPH at the time of impact (to the best of our knowledge). Problem…
Mar 6, 2026 · crashAir bags On March 6, I was on South bound on Rt 29 in Culpeper County VA at the posted speed limit of 55mph when I hit a debris item on the road. I do not know what it was but it impacted the front skid plate and seems to be no other damage from the impact. There was no contact with the bumper, wheels or otherwise. There was no sudden change in speed or loss of control at all. The most dangerous result of this was the deployment of all the airbags (except passenger front airbag, there were no passengers). A police report was filed with Culpeper County Sheriff (case# 2603-0033) and my insurance agency (USAA claim # 016632631-800). The shop where the vehicle was towed (Ace Collision in Madison, VA)…
May 30, 2025Structure › Body I am reporting premature clear coat failure on a 2016 Toyota 4Runner in Magnetic Gray Metallic. The vehicle has approximately 120,000 miles and has been properly maintained, including regular washing and professional ceramic coating. The clear coat on the hood and roof (horizontal panels) is severely peeling and delaminating. The protective clear layer has failed, and the base coat is beginning to oxidize. This is not the result of neglect, damage, or improper care. The condition appears to be premature material or manufacturing failure of the clear coat. I have contacted Toyota corporate and opened a case. I was informed there are currently no other documented cases for gray paint and that…

EA21002 · opened Sep 17, 2021 · openDesiccated Air Bag Inflator Rupture From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled…
